The story of Primo
Ordinal naming survived in Italy longer than almost anywhere in Europe, and it was practical rather than sentimental: large families needed names, and the numbers were there. The census evidence shows Settimo and Ottavio given to genuinely seventh and eighth children well into the twentieth century, and Primo among the commonest. Primo Levi, born in Turin in 1919, is the best-known bearer. The Roman system it descends from used the same words as praenomina — Quintus, Sextus, Septimus, Decimus — and by the classical period those had become hereditary and stopped counting anything, so the modern Italian usage is arguably closer to the original practice than late Roman usage was. The American figures peak in 1912.
Meaning
Italian "first"
From Latin primus, first — the superlative behind English prime, primary, premier and primitive. Roman families used ordinal praenomina, and the Italian practice continues them directly: Primo for a first child, Secondo for a second, and so on through the numbers. Sesto, Settimo and Decimo were still being registered in Italy in the twentieth century.
