The qaf and kaf distinction is one of the things that survives least well in transliteration, and it matters. Qalb, with qaf, is a heart; kalb, with kaf, is a dog. Qalam is a pen; kalam is speech. Arabic speakers hear these as different consonants in the way English speakers hear p and b, and English writes both with k, occasionally with q for the first — but the q is unreliable, since English readers pronounce qu and expect a w after it. Systems that use the letter more strictly write Naqiyah, Qadir and Qur'an; popular spellings write Nakiyah, Kadir and Koran. The result is that a great many Arabic-derived American names are ambiguous about which consonant they contain, which in turn makes the meaning ambiguous. Nakiyah peaks in 2005.

If the source is Arabic naqiyya, pure or clean, the name is spelled with qaf and would more precisely be written Naqiyah. If it is an American formation on Na- with the -iyah ending, it has no meaning. The record does not distinguish them. The -iyah is the Arabic feminine relational suffix, and the initial Na- is also a productive American prefix, so both readings are live.
