It comes from John by way of Jankin — John with a diminutive ending that English borrowed from Flemish — which wore down to Jackin and then to Jack. The evidence was set out in the nineteenth century and has held since. The name was so common afterward that it became the English word for any man at all.

The story of Jack
By the later Middle Ages John was so common in England that a name for it was needed as a common noun, and Jack became that noun: a jack is a device that does a man's lifting, a jack of all trades does any man's work, a lumberjack and a steeplejack are men defined by a job, and the jack in a deck of cards was the knave — a servant. Very few names have been that thoroughly absorbed into a language. The name itself never went away and is currently climbing again, charting in Canada, Scotland and England as well as here.
